About the Role

Community LIFE is seeking a compassionate and experienced Palliative Care Nurse (PCN) to join our supportive care team. Under the guidance of the Supportive Care Manager, the PCN plays a vital role in developing and enhancing care plans tailored to the unique palliative needs of our elderly participants. This position involves both in-home and center-based care, offering ongoing clinical input and coordination to ensure comfort, dignity, and quality of life.

📍  Service Areas: Wilkinsburg, Lower Burrell, Tarentum, and Bedford, PA
🕒  Schedule: Monday – Friday, 7:30 AM – 4:00 PM (occasional flexibility required based on participant needs)
🚗  Mileage reimbursed at the federal rate

Key Responsibilities

• Conduct comprehensive assessments and develop individualized palliative care plans
• Provide clinical guidance and support throughout the care delivery process
• Collaborate with interdisciplinary teams to ensure holistic, person-centered care
• Facilitate end-of-life discussions with participants and families as needed
• Maintain accurate documentation and participate in care planning meetings

Qualifications

Education & Experience:

• Graduate of an accredited nursing program
• Minimum 2 years of RN experience in palliative or hospice care
• Experience in patient education and interdisciplinary collaboration preferred

Licensure & Certifications:

• Active Registered Nurse (RN) license in Pennsylvania
• Hospice and Palliative Nurse Certification preferred
• Valid PA Driver’s License required

Skills & Competencies

• Deep understanding of geriatric and end-of-life care needs
• Strong clinical knowledge and nursing practice skills
• Excellent communication and documentation abilities
• Ability to work independently and within a team-based care model
• Familiarity with local healthcare and geriatric service networks
• Proficiency in Windows-based systems and electronic documentation
